- All applicants must be in the NSHF Community
- If the applicants are outside the NSHF Community but have offices in the NSHF Community they will be considered for a grant
- All applicants must be prepared to present at a NSHF Board meeting.
- All applicants should be prepared for a site visit by the NSHF Board
- All funding must stay in the NSHF Community
All applicants must submit the following:
- Completed application form
- One-page (1) concept paper outlining plans for the funding and how those plans intersect with the NSHF’s mission
- Supporting documents, including a detailed budget, organizational chart, leadership bios, current listing of the Board of Directors and their personal contribution records to the organization.
- Audited financial statement
- Letter of determination from the IRS verifying nonprofit status – 501(c)(3) letter
- Copy of your IRS 990
- Brochure or other summary descriptive material
In addition, please be sure to include the following in the application package either through the application form, concept paper or supporting documents:
- Start date and duration of activities to be funded
- Information about other funding sources for the organization
- History and goals of the organization
Issues to be addressed by the grant:
- Track record in addressing these issues
- Summary of activities for which funding will be spent
- Population and/or area of research to be aided by the grant
- Outline of objectives and anticipated outcomes
- Explanation of outcome measurement
- References from prior funding sources
Review Criteria
The Foundation makes grants only to tax-exempt organizations with 501(C)(3) classifications from the IRS
A number of factors will be considered in determining grant awards including:
- Purpose of the funds requested: Does the organization/program meet the Foundation’s mission
- Potential impact: What is the potential impact of the funds requested on the organization/program? Is there a demonstrated need for this initiative in the community?
- Financial information and organizational stability: Is the organization/program financially stable? Is there strong leadership and direction at all levels? Is there a plan to find additional sources of funding for this initiative? Is there a plan to expand it beyond the life of this particular grant?
- Program history: How will outcomes be measured? How will success be defined and determined?
